Our bridge of care helps to prevent readmission within the critical 30 days of discharge through the management of skilled services and program designed for our client's and their family's unique needs. By bridging the gap between the hospital, skilled nursing facility, and home, we provide a pathway to recovery, renewed independence, and patient empowerment, which create an improved quality of life and overall health. Healthcare can be complicated. Our mission is simple, we advocate for the high-quality care of our patients and their families. Our Medical Social Workers provide assistance during this time of transition. These skilled professionals offer socio-emotional benefits to our families through education and access to available resources, which support the physical, social, emotional, and financial adjustment. Some of the community programs we offer include coordination of a homemaker, caregiver and 'Meals on Wheels.'
