The Colorado Beacon Consortium's mission is to optimize the efficiency, quality, and performance of our health care system, and integrate the delivery of care and use of clinical information to improve community health. The geographic focus of the Consortium's activities includes the Colorado counties of Mesa, Delta, Montrose, Garfield, Gunnison, Pitkin, and Rio Blanco.The Colorado Beacon Consortium (CBC) has been awarded $11,878,279 by the U.S. Department of Health and Human Services to participate in a 3 year health care demonstration project in western Colorado.The CBC will use this funding to improve care transitions, care coordination, efficiency within provider organizations and the health of all patients in the community through the expansion of health information exchange services. Approximately 300,000 people will be positively impacted by this project.. The CBC will improve the health of our community and the quality of health services, and reduce the cost of care to patients, employers and taxpayers.
